Reporting Rare Species

Directions: To report a rare species, first go to the list of species requiring documentation to determine which species require a form. Choose either a pdf or Word format, and save the form to your computer. For pdf files, if you have Adobe reader (the free version), you will NOT be able to save the information you type into the form, you can only print it out and snail-mail it. With other versions of Adobe you can save the form with your notes and email it as an attachment.

You may also type up and snail-mail or email your own account based on the information requested in the form.

Please note: The committee meets in November of each year. Reports are accepted throughout the year, and reports received after Oct. 1 will be considered the following year.
